At OXON Physiotherapy we are committed to ensuring the best standards of practice in all our activities.   • Next of Kin and contact number
  • GP and Surgery name
  • Medical History including medications
  • Social circumstances relevant to the injury

We do not collect any personal information from visitors to our website other than information that is knowingly or voluntarily given by the visitor requiring information or booking appointments from the ‘Contact Us’ section. Anonymous information is collected purely for statistical reasons, such as the number of visitors to the website in a given period with no personal identifiable data. Cookies are used on the website (see below).

The personal data we collect will be used for the following purposes:

  • Identification of clients on booking of appointments
  • Treatment planning based on the medical information provided
  • Record and process the assessments and treatments provided whilst under our care
  • Transfer information to another healthcare professional or insurance company, with consent, if required.
